This chapter details the author's medical practice in Port Elizabeth, South Africa. As a newly arrived physician, he was often asked by other doctors to see a number of patients who appeared to be close to death, and within the first six months saw eight patients who had become paralysed. The author later realized that some of these patients suffered from acute porphyria, which became the start of a twenty-year study.
